What we collect

  • Quiz answers stay on-screen and are not stored locally.
  • Optional intake collects email, phone, and date of birth.
  • Contact form collects name, email, and message.

How we protect data

  • We do not place PII in URLs or query strings.
  • Submissions are sent via secure POST requests only.
  • We do not log personal data to console or server logs.

Third-party sharing

We do not send your information to third parties by default. If the clinic configures a secure server-to-server integration for internal workflows, only the minimum necessary data is forwarded.
